If there is any notification regarding any change in admit card, you must see it and check all the details if anything has changed or not in your old admit card. If no change is there its not at all compulsory/mandatory to download the admit card.

With a score of 425 chances of admission in MBBS in Government Medical Colleges through All India Quota will be difficult. As you have not mentioned your category, I assume it to be Open Category. The cut off for AIQ last year was 550 and more for UR category.
