I had the same problem!! Just enter the DOB as DD/MM/YYYY e.g. 1st Jan 1999 will be 01/01/1999 not 01011999

1) Write an email to neetug-nta@nic.in
2) Subject of email should be "NEET ADMIT CARD Wrong Address"
3) Attach you admit card as well as confirmation page
Within 1 hr they will email you and update your address
