Now wait for the first round of allotment list. If you are shortlisted then you can go for counselling. If you are not selected then choices submitted in round 1 will be treated as null and you have to fresh choice for seat allotment in round 2.

Yes you have to submit Medical Certificate from district hospital approved by CMO of Your district. A Sample format is given in UPSEE Information Bulletin is given at last of that file. You take a photocopy of that format and make a medical certificate according to that. Normally
According to your marks, you may have the chances of admission to Government medical colleges in Rajasthan under State Quota.
You can try for Jhalawar Medical College, Jhalawar.
