Dear Sabir, the form for medical fitness certificate is generated by the respective state authorities and you need to get filled it by any govt/private doc who has valid licence of practicing medicine and has his own stamp.

If you are talking about admission through NEET then you stand a good chance at securing admission in Government medical colleges. Your chances of admission also depend on the state you’re a domicile of, your category, the marks you have scored as well as the state you’re applying

maybe you can write an application and get it signed from the certificate issueing authorities and submit during document verification or else just talk to the document verification authorities about it. they will let you know what to do about it.

Well according to last year's cutoff you need to rank better than 1,20,000 to get into a government medical college in Karnataka (for ST category). 

Since you have score near 2, 20,000. I'm assuming you would have a very difficult chance of getting a government medical college.

There is no such reservation for a particular caste or religion in AMU not even for Muslims however you have to mention your religion while filling the application form.. You just have to sit for the entrance exam and get selected on the basis of merit.. Hope this helps..
