As far as your rank is considered it is difficult to get the top colleges like AFMC Pune, IMS BHU - Institute of Medical Sciences Banaras Hindu University,Shree Guru Gobind Singh Tricentenary University, Gurgaon.
But you chances to get in the colleges LHMC Delhi - Lady Hardinge Medical College for Women,KIMS Karad - Krishna Institute of Medical Sciences,Nair Hospital Dental College, Mumbai,ACDS Secunderabad - Army College of Dental Sciences.
However various factors are considered while deciding the cut offs like number of candidates appearing in the examination, number of vacant seats in college, previous years cut offs and difficulty level of the paper.

The management quota seats are the fixed by the colleges and the candidate selected for that seat are giving the higher fees which are ansked by the institutions and it is very high amount which are taken by candidate but the paid seat quota has selected the students on the basis of their rank and the fees are lower as compared to management quota.

Hope you are doing great as per the question asked by you, as if you are not able to submit E.W.S category certificate at the time of counselling then you would be considered as general category student and you would not be able to claim the category quota seats for the admission in NEET MDS .
