As you have not mentioned your category that's why it would be little tough to predict you anything but here i am assuming your general category so with general category you have little chances to get government colleges but if you belong from any other category

i think proforma is not needed as the information bulletin is not updated......it the same (carbon copy)  as 2019 neet information bulletin. take only what the hall ticket instructions say.

According to the dress code of NEET 2020  t-shirts are allowed but full sleeve t-shirts are not allowed so, make sure you t- shirt doesn't have full sleeves and coming to the color yes, you can wear black t-shirt but i recommend you to wear only light colors
