Listen, Instead of Aadhar card you can use your another Identification Proof like your PAN card , Driving License , Voter ID card etc.
BUT, If you wants to add your Surname in your Aadhar card it's also Possible .
You can visit Aadhar Enrolment Center nearest branch , they will ask you to fill a form and mentioned the important details, After submition the form at the same time they will add your surname and allot you a Virtual Identification Number there after you will get your Aadhar card in next 14 days on wards.
But make sure you can change your name once in your Aadhar card so try to fill the given form Precisely.
Although you can use your another Identity proof during NEET Examination.

The National Eligibility Entrance Test (NEET), formerly the All India Pre-Medical Test (AIPMT), is the qualifying test for MBBS and BDS Programmes in Indian medical and dental colleges. Hope you are doing well . You just have to upload the the page on which your roll no. , Registration no. ,Centre name and all other relevant information is mentioned i.e. the first page . Rest there was a page to paste your postcard size photograph which you submitted to the centre while appearing for the examination and the other two pages left were the guidelines and rules to be followed on the examination day .
So , you just need to upload the first page .

Do not worry if your original neet admit Card is lost, all you can do is to visit the official website of NEET UG and then click on the admit card option on the portal , it will ask for the basic registration details, so just fill in the details and click submit/ download. Hence you will get your original admit card downloaded.

For participating in the online counselling process for NEET 2021, submitting the admit card is necessary. Other required documents include class 10 marksheet, class 12 marksheet, NEET scorecard, ID proof, provisional allotment letter, caste and PwD certificate (if applicable)
