This is not certain now that you have to deposit this certificate while registering for NEET.
But this is nothing but similiar to gap certificate which candidate used to deposit at the time of counseling mentioning the years he/she dropped after class 12th before giving NEET
This is a simple certificate, you can get from your city with the help of lawyer or any legal authority.

Biology can be filled as an additional subject but remember additional biology does not mean the optional subject mentioned in the form during registration. You can fill the name of the school from which you have done PCM with 10+2 in NEET application form and biology can be filled as an additional subject taken.
There is no need to fill the name of the school from which you have done biology as an additional subject.

The date for Neet 2021 application form release hasn't been announced yet but yes neet 2021 registration website has been launched by nta at neet.nta.nic.in . For the release of application form ,you need to keep an eye on its official website i. e ntaneet.nic.in. or alternately follow our page for updates ,the link for which is provided below
Once, the application form gets released you will be able to fill it in online mode at the official website of neet i. e ntaneet . nic . in and for that you need to go through 5 steps i. e
neet registration to generate your login credentials,
then fill the form,
and after that you need to go for uploading your documents,
then pay the application fee
And finally you should take the printout of confirmation page

There is still an impending uncertainty when it comes to NEET 2021. Under the current Corona virus pandemic many prominent social workers, administrators and politicians including the chief minister of the state of Tamil Nadu Mr. M. K. Stalin have requested for the entrance exam to be canceled and instead the admission process be based on 10+2 level marks alone. However there is yet to be any declaration on the matter. Hope you are doing great as per the question asked by you the name in 10th report card is your final name that will only be considered every where in case you have changed your name then you should contact you respective board regarding it. Any discrepancy will lead in cancellation of form so you may solve it as soon as possible.

Hope you are doing great as per the question asked by you yes you can certainly claim your O.B.C category in N.E.E.T if you have certificate. For securing a seat under all india quota you need to have a central O.B.C certificate and for admissions under state quota you need to have O.B.C certificate of state then only you can claim reservation.
