Hi,
Since you have all the required documents of yours you will not need your father's documents as only candidates documents are required. But you should always keep your mother's documents because one of the guardians documents should be there. In some places like income you need to provide the income tax details of past 1 or 3 years during the time of admission in college.
I suggest you for your betterment keep one of your parents documents ready with you as you might need them.

You need to choose the code 1.
This is because ,
code 1 is for 12th board 2021 ( doesn't matter whether it's regular or private candidate) appearing students or have already appeared for 12th board 2021 examination but results are awaited i. e results are not declared yet.
Here's a short brief of Neet codes :-
--------) Code 1 :- code 1 is for class 12th board 2021 examination appearing students with physics, chemistry, biology and English or have have already appeared but results are awaited i. e have not been declared.
--------) Code 2 :- code 2 is for all those who have passed their 10+2/higher /senior secondary/ Indian School Certificate Examination with physics, chemistry, biology and English
--------) Code 3 :- code 3 says it is for those passed their intermediate / pre- degree examination with physics, chemistry, biology and English ,your physics, chemistry and biology should also contain practical exam.
--------) Code 4 :- pre - medical degree with Physics, chemistry and biology.
--------) Code 5 :- code 5 is for all those who are doing graduation and have all the three of Physics, chemistry and biology in their graduation.
--------) Code 6 :- code 6 is for all those students who have passed BSc with at least any two of following subjects :-

For now only the registration website has been launched for NEET UG 2021.no dates have been released. It is expected soon. NTA will soon announce the application form date at neet.nta.nic.in.
NEET 2021 exam date is August 1,2021.

Till now no dates have been released for NEET 2021 Application form so we have to wait for any official announcement regarding it. For now ONLY THE REGISTRATION WEBSITE HAS BEEN LAUNCHED for NEET UG 2021 but there is no update regarding NEET UG 2021 Application form.

NEET 2021 Application process :-
** Very first step is to visit https://ntaneet.nic.in/Ntaneet/Welcome.aspx and fill out the application form, then on first menu you have to do registration using your basic details such as name, father's name, mother's name, contact details and so on.
** Now on second menu item you will have to fill details like your academic performance, address, ID type, medium of question paper and so on.
** Then on Next menu item you have to upload some images such as your passport size photograph (10-200 Kb in JPG format), Signature (4-30 Kb in JPG Format), Class 10th certificate (100-300 Kb in JPG Format), Left-Hand Thumb impression (10-50 Kb in JPG format) and postcard size photograph (50-300 Kb in JPG Format).
** Then you have to proceed for payment of application fee which can be done only in online mode either through Credit/Debit card or UPI or NEFT/RTGS.
** Then after you have to take the print-out of the NEET Acknowledgement receipt.

You cannot the choose or select college name or the college you want to apply for while filling the Neet application form .
This is because,
There's no option for the same while filling the form. You can select the college you want to apply for only after qualifying Neet examination while participating in counselling. You can participate in in both mcc counselling and your state counselling and during the counselling you have to fill the choices of colleges you want to get admission, in your desired order of preferences and then seat allotment list is released, in which seats are alloted to students on the basis of rank in neet, category of student etc.

Recently, on 2nd June , neet 2021 registration website has been launched by nta at neet.nta.nic.in ,so the application form is expected to be released soon. Once, the application form gets released you will be able to fill it in online mode at the official website of neet i. e ntaneet . nic . in and for that you need to go through 5 steps i. e
