Looking upon this years cutoffs you have indeed qualified the NEET 2021 examination with 128 marks and also considering the fact that you are from the OBC category,
The qualification are as follows
Neet 2021 qualifying cutoff for different categories is given below:-

OBC 40th 137-108

So,
An obc/sc/st category student requires at least 108 marks, to qualify neet 2021 and for general category at least 138 marks is required to qualify Neet 2021
But the point here is whether you should participate in the counsilling process or not. firstly the AIQ will start that is the All India Quota which conducted by MCC/DGHS .
but there are only 15% seats available for the students to apply in the colleges all over the country. so your chances in this particular counsiling process are quiet low.
But do not lose hope there would be the State quota counsiling process for 85% seats where you could try and apply in some of your state's private medical college.

Your candidature will not be rejected on the grounds of different address in NEET application form and Aadhaar Card. This should be your least concern because Aadhaar card is to verify your identity and not your address. Your present address can be different from the address on the Aadhaar Card. You need to fill the address for communication purpose, if any. You do not have to worry about this and there is nothing you are required to do regarding this. It is assured that your candidature will not gets rejected due to the difference in address on Aadhaar Card and NEET Application form.

Hope you are doing well. See,it's very unlikely that that someone will not be able to participate in the Counselling process just because of changed address in Aadhar Card. As NEET and the Counselling process are conducted by two different authorities and NEET exam is over now ,the only job left is that of the Counselling authority.
Whenever you'll be registering for the Counselling process and during admission and document verification in the college you'll be allotted,you must provide your new address on the Aadhar card . However,when you'll report into the college you'll be allotted,you must carry both your aadhar cards with the old and new address in case it's needed . But while providing information during registration and admission into college,you should provide your new address.

As per your query I would like to tell you that you dont need to worry in your case because address the generally asked to maintain the authencity of the location , moreover you have done the good by updating your adhaar card at the time of verification of documents show the originals to them ,they will verify you successfully

The permanent address is must and should to apply neet application , so you can give or add any other proof which is having Your permanant address . Or else you can update your temporary address to permanent in your aadhar before you are going to apply your neet application

The Honourable Court has mentioned that for Admission and Entrance exam purposes, Aadhar cannot be a mandatory Id proof. You are required to fill the proof of identity details , if you don't have Aadhar card you may fill the detail. Supreme Court has passed an order stating that Aadhar Card is not mandatory for Academic purposes. So you can upload another Id proof like your Driving license or Passport. Aadhar Card is not mandatory. ls of other identity cards like voter id card, PAN Card, Driving License, passport.

There are no such instructions available, as such. So don't worry you can provide any image as you want in the counselling, provided you must be sure about the details you fill are correct. The only problem you can face is only if you fill wrong details except that with the photo issue you have mentioned, you have nothing to worry about and no one gets rejected for such issues. So stay cool!!
I would recommend you to click a recent photograph with the same attire if possible and use it or you can simply use any other recent photographs. There are no such hard rules regarding this.

Hope you are doing well . See,if you changed your address in your aadhar card ,you should carry both the Aadhar cards while the document verification in the college you'll be allotted. The address that you already entered while filling the application form of NEET UG-2021 cannot be changed now .
When you go to your allotted college for document verification,there you should produce your new aadhar card with the changed address (though you should carry both old and new ones) and inform the college authorities there itself . There ,whatever instructions will be given by them you'll have to follow that but most probably your new address will be entered into their records.
