Yes, you can request for your second practical date as soon as possible otherwise your result would be declared as SYCP (SUBJECT YET TO BE CLEARED In Practical) and your result would be failed even if you passed in Theory .

Students passing class 12th from NIOS( National institute for open schooling) are required to full same details in class 11th schooling as in class 12th

For example- state of schooling of class 11th and name of school both will be same as your class 12th NIOS
