For Class 11th preparation, I would suggest you to refer the NCERT textbooks first. Other books which you can refer are Pradeep's for Physics and Chemistry. Also, SL Arora is a good book for preparation of physics. Concepts are well explained in this book.

YES. CUSAT started offering 5 year integrated BS-MS Courses in physics , Chemistry, Mathematics and Statistics from the year 2018. You can also end the course after 3rd year with a B sc degree if you want to go somewhere else for post graduation.
