No you don't need to wear the school uniform for your NEET exam. You can wear normal, civil comfortable clothes and your mask and gloves which is compulsory.
You should wear only half sleeves as full sleeve clothes are prohibited. Jeans with big pockets are also not allowed so

If your school uniform shirt has long sleeves, then you can't wear. Moreover, in school uniform you wear belt and shoes but these are not allowed in neet. So, in short you can't wear school uniform.
