Sorry,You have scored very less marks in NEET exam. According to previous years cutoffs you will not be able to get any government medical college as their cutoffs are very high enough. You can get low level private medical college but their fees will be too high.
I would suggest you to drop a year and study hard for next year or look for other options.

Since you have not mentioned the course you are looking for, I am providing you with a general answer. You need to fill seperate forms for all colleges or universities and if you are applying for more than one course in the same university, then also you need to fill seperate form. In the previous years, the cutoff of sharda university has been around 320.
That cant be said for this year as the cutoffs depend on the difficulty of the paper, number of students appearing and qualifying the NEET exam.

Yes, you can. The application form is made available from last week of April (usually from April 28) and the last date is in June. So, there's a lot of time. The admission is done through JEE Main, UPSEE or SUAT (Sharda University Admission Test). Good luck.
