You need to choose code 3 as you have passed from karnataka board

This is because,

Karnataka board i. e The Department of Pre-University Education, Karnataka  offers Puc

And,

code 3 says it is for those passed their   intermediate   / pre- degree examination i. e pre university (  Puc) with physics, chemistry, biology/biotechnology and English ,your physics, chemistry and biology/biotechnology should also contain practical exam.

Yes, with PCB you are eligible for BSC chemistry.

This is because,
Maths is not mandatory  for bsc chemistry.

For example,
To be eligible for Bsc chemistry in Chandigarh University, Chandigarh, you need to pass 10+2 examination in Science Stream with

To be eligible for Bihar Combined Entrance Competitive Examination Board Lateral Entry Test (BCECE LE) engineering. Candidates must have passed a diploma in the required stream with a minimum of 45% marks. The minimum percentage marks for reserved category candidates is 40%. There is no age limit but if you do not clear the above eligibility criteria you will not be eligible for the exam. You have not mentioned the course you are interested in. For admissions in UG courses in Institute of Law, Nirma University, Ahmedabad are done on the bases of CLAT exam. The Common Law Admission Test (CLAT) is a national-level law entrance exam for admission to 22 national law universities and more than 70 affiliated colleges. The exam is conducted by the Consortium of National Law Universities (NLUs) in offline, pen and paper format.  The candidates must have passed Class 12 (10+2) or an equivalent examination from a recognized board or institution. As per the eligibility criteria specified in IPU Admission 2022-23 Brochure the eligibility criteria to get admission to B.Tech Biotechnology is that a candidate must have passed 10+2 or its equivalent with minimum aggregate of 55% marks in Physics, Chemistry, Mathematics and Biology/Biotechnology, The candidate must have passed in each subject separately. Candidate must additionally have passed English as a subject of study (core/ elective/ functional) in the qualifying examination that is 10+2 level. The admission is based on merit in IPU CET. If you haven't studied Mathematics along with Physics, Chemistry and Biology/Biotechnology then you are not eligible to apply for IPUCET to get admission. Yes, you are eligible for mbbs /neet even if you have hearing loss provided it is less than 40%

This is because,

• Students with hearing impairment  less than 40%  are eligible for mbbs
• Those with equal to or more than 40% of hearing impairment are not eligible for mbbs
• There's no pwd quota for hearing loss .

You are eligible to do B. tech (biotechnology) from all the govt institution except IITs and NITs as you have done your 12th from PCB , so ,you are not eligible to appear in IIT entrance examination ..Apart from those (IITs and NITs) you can do you BTECH.
