Both the options are best, but after doing MBBS you have clear the MCI screening test to practice in India. Nowadays only with MBBS there is not much height. You have to do Pg course. However, for BSc biotecholongy there is not such screening test and Many companies will offer the job if you have aborad degree

Well ielts is an exam that shows your English speaking, reading, writing and listening fluency. Any country where the language of medium is English will require you to show some fluency in the language and that is why IELTS is preferred but not everywhere because India also has English as an official language.
