If your name is wrong in your 12th mark sheet you could carry an affidavit stating that the name in the 12th mark sheet is wrong and the one on your class 10 mark sheet is the same. There are some chances of your application not being denied

The best of four subjects will be English, Physics, Chemistry and Bio/Mathematics depending on which one has better marks. Please don't add physical education in the best of four. To know more about the best of four calculations, please refer to the link provided below.
