You dint quite provide the clear name of the exam, I assume it is NTSE, I am so sorry to inform you that For Physically Handicapped (PwD) people cutoff is 49.33 for NTSE. So probably you wont be able to qualify with 40% of marks.

First of all don't get demotivated. There is still a while for JEE Main. So, I would suggest you to focus on basics. Also practice a lot of previous year papers or rather I would suggest you to by heart them.

You will require your 10th and 12th marksheet of a recognised board. Then you will also need your birth certificate along with your Domicile certificate.
