First of all you don't need to write your date of birth on your photos but yes you need to get your name and date of photo written on your photos which you will be uploading while filling neet application form.

Don't worry if you entered the wrong address,you can change it during the correction period that is from 15 January,2020 to 31 January,2020.It will release by NTA and you can simply go in NEET official website https://ntaneet.nic.in/ntaneet/welcome.aspx and can correct your documents.
