It is not clear from your question what type of certificate you want to know about.
For jee advanced, A valid GEN-EWS certificate as per the prescribed format, given on the JEE (Advanced) 2020 website, issued on or after 01-April-2020, should be uploaded for availing the benefits of GEN-EWS category.

At a neet score of 450 and rank 132015 general category you have less chances at government institutions and private instituions of madhya pradesh as the cutoff is around 471+.
you can apply for private institutions. you will be eligible only for open seats to all under private institutions to all states.
you can easily apply to get into any private medical college in india but however we recommend you to take admission in private medical colleges of karnataka or maharashtra as the fee is quite less there around 6-7 lakes per annum and also they provide with all the latest equipments and facilities required during mbbs course.
Also maharashtra and karnataka have maximum no. Of private colleges in the country so you have more and better options to chose from.

Yes you will get either SPA Delhi or some top NIT.Your Rank should be around 2000, a general observation.
You can try for NIT Jaipur, MANIT Bhopal, NIT Raipur etc. To know your admission chances in JOSSA & State counseling you can use JEE Main

As you might have an idea that this year cut off gone rise 30-40 marks above then previous year as topper Mark is itself 19 marks above then previous year. As per your marks there are confirm probability that you will get govt college. Last year cutoff in mopup round for government college in bihar for your category was
