At a neet score of 374 you have no chances at state quota or all india quota the cutoff is around 530+ and also this year cutoff has increase significantly.

Your question isn't clear as it doesn't mention which certificate you are talking about. If you wish to avail your caste reservation, then you need to upload your caste certificate during counselling. If you wish to avail colleges with State quota seat (85%) then you need to upload your domicile certificate (the state from which you passed your 12th boards).

Few days earlier UGC issued a statement which states that no university can grant affiliation to more than 300 colleges if they do so a penalty will be imposed on them and other strict actions would be taken. In 2019 a candidate belonging to OBC-NCL with 638 marks secured seat in SMS Medical College, Jaipur under 85% state quota. If you belong to OBC-NCL category then you have decent chances of getting seat in this college. If you belong to OBC-CL then you are considered as candidate under general category the cut off for general category was 641 considering this you have decent chances of getting seat and the cut off under 15% AIQ quota for general category was 655 marks for admission int SMS Medical College, Jaipur.
