You can prepare for entrance exam while doing job but then you have to leave your job to attend college to get a degree. You cannot get degree while doing job as for becoming a doctor the curriculum is difficult and you have to give your 100% focus to it.

Mumbai University also known as the University of Mumbai is a private State University situated in Maharashtra. The University offers bachelors, masters and Doctoral courses, as well as diplomas and certificates in many disciplines like the Arts, Commerce, Science, Medical and Engineering and has several notable Alumini passing out through the college.
In order to fill the college form, you have to follow some simple steps- First you have to visit the Admission portal and select your college and your prioritised Branch and click on NEXT. After that you have to upload all the Documents asked and then read all the information and guidelines provided, and click on Submit. Your Application Form will be submitted!!

However I'm very afraid to inform you that despite being reserved category canditate, with your ranks and percentile it's too much difficult to get the opportunity of getting admitted in any kind of renowned NITs and IIITs in India (except NIT Srinagar and NIT Sikkim). Here you don't have mentioned your state in your question, so it's quite difficult for us to make any decision regarding your admission chances in State Counselling procedure. Still after analysing the previous year's cut-off informations, we can conclude that you've enough chance in several top NITs like NIT Raipur, NIT Meghalaya, NIT Goa, NIT Jalandhar, NIT Jamshedpur, NIT Manipur, NIT Agartala, NIT Hamirpur etc through 85% Home State quota (i.e. if you belong to the respective states of those NITs then only you can grab a seat in these NITs). Moreover you've a definite probability to get admission in different State Government Colleges and other private engineering colleges in your state, hope that you have already applied for State Level Counseling which has already started.

You can apply to b category in any colleges by just having online printed form or directly you can fill all the required data of class 12 results and mains percentile ..or you can apply form for college own entrance exams .

Assuming you are mentioning this rank from JEE Advance, this rank is a good rank considering you belong to the OCB Category. While the increasing competition makes it tough for you to get into the top IITs in the top courses like CSE or EEE in universities like IIT Mumbai or IIT Madras, you might be able to get into some lower ranked IITs such as IIT Hyderabad or IIT Tirupati or other newer IITs. Some older IITs are also possible like IIT Roorkee, IIT Indore etc.
