It's quite difficult to predict that whether you will get seat or not as cut off marks for medical colleges keeps changing every year depending upon various factors like number of students that appeared in the exam, kind of marks obtained by them , toughness level of the examination etc.
But when we go through previous data then it suggests that with 503 marks you hold a chance to get a seat in GOVERNMENT MEDICAL COLLEGE

Well anyone who has completed a MBBS or BDS(special case is a Ph.D) degree can add Dr. as a prefix before their names. MD Emergency medicine is a specialization course that is done only after one completes MBBS.
So, emergency medicine physicians do use Dr. as a prefix before them. In fact, they can do it right after they completed their MBBS.

If you are pointing to Shri Vishnu College of Pharmacy, Bhimavaram. Then, yes you have got pretty good chances to get a seat for B.Pharma in your preferred college.
The last year closing state cutoff rank for B.Pharma in this college was 67,247 for MOC stream. If you are from BIPC stream it may be little hard for you to get a seat. And there are about 100 seats for B.Pharma in this college.
