1. Call Letter/ Hall Ticket/ Admit Cards of NEET-UG 2021 exam provided byNTA,
2. Score card issued by NTA
3. Date of Birth Certificate (if Metric Certificate does not bear the same)
4. SSLC Certificate (for date of birth) and Marksheet
5. HSC Marks
