See as of now there is no upper age limit for NEET, as the decision for the same is pending in the Supreme Court, hence until then as long as you fulfill the eligibility criteria as mentioned below you can appear for NEET;
Attained 17 years by 31st December of the appearing year.
Passed or appearing 10+2 from a recognized board with Physics, Chemistry, Biology/Biotechnology and English as core subjects.
PCB Aggregate inclusive of theory and practical needs to be achieved against one's respective category-
- UR - 50%
- SC/ST/OBC - 40%
- PWD - 45%
If you fulfill the aforementioned eligibility criteria you can appear for NEET, and for this year, then as of now NTA hasn't released the schedule officially, also NEET 2021 counselling is ongoing though it's being expected that by next week the details may get released, however to know the exact details regarding this, you may follow the official website at https://neet.nta.nic.in/ to know the latest information regarding this,

Yes, you are eligible for NEET examination even if you have passed 12th in 2016.
This is because,
There's no attempt limit for neet, also there's no such boundation that students should have passed 12th within certain period of years, so you don't need to worry at all.

Here's the eligibility criteria for NEET :-
-----------) Qualification required :-As per Neet eligibility to be eligible you must be either appearing for 12th board examination or should have passed 12th board examination from any recognised board with physics, chemistry ,biology/biotechnology and English as core subject.
†††Important Note :- Students with Biology as an additional subject are also provisionally eligible for neet until Supreme Court gives its judgement over it.
-----------) Percentage required :- You must get

at least 50 % in aggregate of PCB i.e Physics, Chemistry and Biology/Biotechnology in 12th examination if belongs to general category

at least 40% in aggregate of PCB if belongs to st/Obc/ Sc
-
at least 45 % in aggregate of PCB in case of pwd candidates to be eligible.
So, if you meet this criteria then you are definitely eligible

Lower age limit:- to be eligible for neet one has to be at least 17 years old or more by 31st December in the year of appearing for the examination .

Upper age limit :-The upper age limit is 25 years as on the date of neet examination and there's relaxation of 5 years for obc ncl/SC/ST /PwBD students means upper age limit is 30 years for these categories.
†††† But since 2018, the case of upper age limit is pending in supreme court , and so there's no upper age limit as of now until supreme court gives its judgement over it.
