Hey halder ,You have to visit to a doctor for your medical certificate. Only MBBS/pg doctors can make the medical certificate of candidates.
Doctor will take fees to make that medical certificate.

You need to go to a local doctor and ask him to make a medical certificate of you in the format prescribed in JOSAA business rules. I will recommend you to take out a printout of medical form from JOSAA business rules and ask an authorised doctor to fill it.
