Well the most standard exams like neet still maintains the traditional pen and paper pattern.. But there are variety of institutions which conducts the exams online..Although even in those u are given a venue like a school or something, where the exam is conducted under strict vigilance..some examples are:
AIIMS
JIPMER
MANIPAL
COMEDK
etc
As on date, examination like NEET-UG continues to maintain the tradition of conducting an offline mode of examination (pen-paper based).
However, examinations like AIIMS and JIPMER have switched to online mode of conducting examination and it is perhaps the only available method for conducting examinations as on date.
Other deemed universities like Manipal and SRM university have also started conducting admissions through the All India counselling conducted by DGHS.
Government run institutions such as AMU and BHU have also started conducting admissions in a similar manner, you only need to apply for these colleges after the results of NEET are declared.

The NEET exam is not online-based But many other PMTs are - eg Manipal The test centres would obviously be centralised. Online test taking tools are available for practicing questions to boosten the exam prep. I highly recommend it. Refer to sites such as Aakash for the same. Majority of Undergraduate examinations are written exams, you have multichoice questions which you need to tic. You will be given an OMR sheet which needs to be filled using a blue or black pen.
Only AIIMS- All India Institute of Medical Science has an entrance exam which is conducted online.
Also the applications for entrance exams can be done online, but the exams happen in the examination hall, mostly via an OMR sheet.

If u mean online exams wrt mock exams then any app like career360,accettop,prexam,careerorbits,prep.youth4work will be sufficient to give analysis ..whereas NEET-UG is OFFLINE n requires you to go to Ur respective centre but on the other hand AIIMS is online .
