Can JEE advanced only be the given in the year I first appear for 12th and the following year. What if I fail to clear MAINS, will I get two chances or I fail MAINS it would mean one chance for ADVANCE lost

According to your question, yes that means that you have lost one chance for JEE Advanced because you can appear for the examination in the year you have passed class 12th and the next year.
