It is time to use the short notes that you made during the intense preparation for the past one or two years. Check the important topics, formulae and in short revise. Ideally analyse while you revise. This will help you to strengthen any point you are weak in.
STOP studying new chapters completely. It will NOT help at this moment. Revise the basic concepts and formulas, especially for the areas that are your strongest (yes strongest, not weakest). Make sure you get everything right in these.
Try some practice tests. A lot of them are available online with solutions. You could try the previous year Question papers or other sample papers at this time. The purpose is to put yourself through the 3-hour exam routine (preferably from 9:30 to 12:30) multiple times so that you feel comfortable on the day of the exam.

Physics-
Make sure your concepts are clear. Make important notes, write down all the important formulae, shortcut and tricks. It will help you to revise a day before the exam. Practice as much as MCQs for your practice. Don't try to attempt complicated questions in a last month.
Chemistry-
You can take help of NCERT book for chemistry. Try to attempt all the questions and exercises which are given at the end of the chapter. You can also solve previous year papers questions as well for your practice and to understand the level of the exam.
You can also refer other books other than NCERT for your preparation but NCERT should be your first priority.
Mathematics-
For Mathematics, you need to practice more and more. As much questions you will solve you will build a command on the subject.
