I am average student and i want to do b.tech .which form should i fill ?

This depends on which type of college you want to take admission.

Some Colleges are give direct entry based on 12th marks. But if you want to study in good college you need to give jee mains exam which is most common engineering entrance exam. Mostly college allow the score of jee mains.

Also there is some particular college which conduct thier own entrance exam like viteee, bitsat,SRM

To begin with, you must definitely appear in JEE Main, as some 26,000 odd seats are filled in NITs, IIITs, and CFTIs based on JEE Main scores. It is because with a bit of luck and some hard work, who knows you may end up getting a good seat.

Also, even some good students may choke under the pressure. So, if you can hold your composure and maximize your abilities you can punch much above your weight.

I would advise you to appear in JEE Main also because, it will make it easier for you to attempt other exams - like IPU CET, state engineering exams, exams conducted by private institutes like VITEEE, MET, SRMJEE, etc.
