With an AIR of 3974 in NCHM JEE and General category , getting into the top 5 IHMs will be difficult, but you still have some chances in a few of them depending on the counselling rounds. Based on recent cutoffs:

So, if by "top 5 IHM" you mean Pusa, Mumbai, Bengaluru, Kolkata, and Chennai, then Chennai is the one where you have the best chance. The other four would be quite difficult with a rank of 3974.
