List of Documents Required for NEET-UG :-
The candidate has to bring their original documents (along with attested photocopies of certificates) for admission in MBBS BDS course.

Call Letter/ Hall Ticket/ Admit Cards of NEET-UG  exam provided by CBSE.
Result/ Rank letter issued by CBSE.
Date of Birth Certificate (if Metric Certificate does not bear the same)
Class 10thCertificate (for date of birth)
Class 10+2 Certificate
Class 10+2 Marks Sheet
Eight (8) Passport size photograph same as affixed on the application form.
Provisional allotment letter generated on-line.
Proof of identity.

If you don't produce original documents during verification of documents, you wouldn't be allowed to proceed further. You have enough time to get original certificate. Apply for it as soon as possible.

Yes you will be required to provide Caste Certificate

The application forms are available either online or from the concerned local office in the City/Town/Village, which is usually the office of the SDM (Sub-Divisional Magistrate) or of the Tehsil or Revenue Department.

Yes. You need to produce the caste certificate,which is issued by the district magistrate  of your place where you reside,of your domiciled state.. without that you cannot avail the benifits of reservation.

Yes of course. You need certificate of your category in order to avail reservation benefits (if any). You need to submit a photocopy and also you will have to show it in original at the time of counselling.
