You have not mentioned your category any where. Neither have you mentioned your state specifically. Nor have you mentioned the course in which you are seeking admission. So it's really difficult to predict anything.
Keeping the general trends in mind ,it's impossible to get admission in any government medical college with this rank. You might hopefully get a government seat for ayush courses which include courses like bams, bhms, bums etc.

As you have secured 221446 all India rank it is ok
So according to the analysis of data .the one with your rank is difficult to get in the government college as the competition in the government college is very high for the greater studies
