I have already appeared for manipal phase 1 exam and I want to appear for the second attempt as well ..do I need to register or fill the application form again ?

For taking admission in Manipal University for BTech courses candidate has to appear for an interested that is call Manipal entrance test. Entrance test is divided into phase 1 and phase 2 held twice in a year. If anyone appearing for phase 1 not have got success in phase1 then he  may  appear  again in the phase 2 of Manipal entrance test for taking admission in B.tech courses. The entrance test is taken for  the same course and same pattern  and same timing consist of the same pattern of examination both in phase 1 and phase 2 exam.

as per you have mention that you have appeared on face 1 examination and if you think that you will have a great score in Phase 1 then you don't need to fill for phase 2 again.
