I have got 77 percentile but getting very low grades in school due to my father's tranfer in middle? How can i atleast pass maths and che.

Chemistry:
Chemistry(100 marks)==> Practical(30 marks)+theory(70 marks).
To pass you must have to score atleast 12 marks in practicals and 23 marks in theory.
To pass chemistry paper open you ncert books and solve all the exercise questions of these chapters
1 Chemistry in everyday life----->approx 0.5 hour----->3 marks.
2 Polymers------------------------>approx 1 hour--------->3 marks.
3 Biomolecules-------------------->approx 2.5 hour-------->4 marks.
4 Compounds containing nitrogen->approx 3 hour----------->4 marks.
5 surface chemistry----------------->approx 4 hour----------->4 marks.
6 haloalkane and alknes------------>approx 4 hour------------>4 marks.
7 Coordination compounds---------->approx 4 hour------------>3 marks.
If you give the time to each chapters as mention you can score 100% as these chapters are pretty simple and most ques come directly from ncert exercise.But try to read other chapters that will help you to increase your marks.
Maths:
Try to focus on these chapters -:
1 Matrices and Determinant(Algebra)------>approx 8 hours----->13marks.
2 Linear Programming----------->approx 3 hours------>6 marks.
The above chapters are easiest in the entire syllabus.So try to score maximum in these chapters.
3 Vectors & three-dimensional Geometry----->approx 9 hours----->17 marks.
These chapters(mention in point 3) are easy enough to score around 10 to 12 marks if you study throughly and if you are lucky enough you can score 17 also.
4 Continuity and differentiability---->min 8 hours-------------->8 marks.
In continuity and differntiability former is easy to score as they generally ask for a proof so you can get step marks in easily.
5 Probability------->approx 8 hours----------->10 marks.
if you give min 8 hours you will score at least 5-6 very easily.
**plus please learn all formulas and see some solved examples of other chapters because if you can get marks on writing formulas and steps.

If you just want to pass your exams, just focus on chapters that have higher weightage. Read those chapters on NCERT first and then watch the videos of that chapter on YouTube. There are very good educational channels on YouTube that explain the concepts of Maths and Chemistry very well. Reading the chapters from book and then watching relevant videos of that chapter will ensure that you understand the concepts well and it'll help you pass the exams.

I can understand your situation. Don't be nervous just focus your and settle down and plan your studies you'll easily score high.
For Mathematics try to solve NCERT first whole heartedly and practice as much as you can then after take the previous year papers doing that much will help you score a very good score.
For Chemistry first study the chapters and after that study the NCERT solutions from any reference book then after that go for previous years.
Doing this will definitely help you to score good. But one thing keep in mind to relax yourself because if you hesitate you'll not be able to concentrate properly.
