You can apply for a new one. The form is available on CBSE website and also mentioned below. You will have to fill the form and get it signed by your principal. You will have to pay some amount in form of postal order that will be mentioned in the form.

A FIR of lost document would be required.

Now, either you can go to CBSE office by yourself or you can send it via speed post.
