I registered for JEE main 2020 january exam but I didn't appear so now I want to register for april 2020 exam. Should I login with the old account or should I register again as a fresh candidate? please help me I am confused.

The best way to check it, if you have any sought if confusion is to apply and see. Usually, there's no such issue. If you didn't appear for Jee Mains in January 2020, no problem. You can register from the same id for Jee Mains to be held in April 2020. If you face any problem, try registering from another email id. If you have any further confusions, you can visit the official Website and contact the helpdesk. They will definitely help you with this.
