Hello aspirant,
Since you have completed your class 12th with physics, chemistry, biology and English as your main subjects then you are definitely eligible for appearing in NEET 2021.
However, since you are not pursuing the Bachelors degree in any of the science courses and since you have only completed class 12th with science stream, then you have to opt for code 02 which says that you should have completed class 12th with the required subjects as I mentioned above.
