if two student has same marks in neet(UG) and suppose seat only 1left then to whom the medical college give entry ???

Actually , seat allotment is through your rank as the problem with marks is that two or more candidates can have the same marks but no two candidates can have the same rank , not even if both candidates have got the same marks.
This is because,
When two candidates have the same marks then tiebreaker criteria is used to provide them rank
-) the one who obtains higher in Biology gets better/lower rank
b) if there is still tie then the one who has got more marks in chemistry gets better rank .
c) if there is still tie then student with less number of incorrect answers gets better rank.
d) if the tie still persists then the one who is older in age gets better rank.
For example,
In neet 2020 both rank 1 holder and rank 2 holder had got 720/720 marks but the rank 1 was older ,thus he got rank 1.
So, to answer your question,
If only one seat is left and both the candidates have the same marks and then the one having the lower rank will get the seat
