To the right bottom of the page, you will see a heading called 'Certificate'. Under that, younwill find a PDF format of SC/ST Certificate. Please download that, fill it accordingly and sign it. And then get it scanned in a PDF Format and upload it to your application. It should work this way. And you can select only those options that are applicable for you.
