In my aadhar card and birth certificate my surname is mentioned but in the school certificate my surname is not mentioned... I have applied for the addition of my surname legally and already have an affidavit... should I mention my surname in the jee mains exam form???

Since you already have an affidavit you can add your surname in JEE exam form. Be sure to carry the affidavit with you at the time of counselling.
