Yes, you have to go for a health check up for a JOSAA certificate.You can take the printout of page 53(Annexure 8) from the business rules pdf from the official josaa website and take its print out.

Later you can visit  any government hospital and ask them for check ups written in the format of the medical certificate in JOSAA's BUSINESS RULES.

Plz note that you have to get your check up done  in gov hospital only  else your certificate will be invalid.
