Is it compulsory to print medical fitness certificate on our doctor's prescription or is it ok if we just write on the given form?? Please

It is better to give separate printed fitness certificate as compare to written one.

The doctor will have his letter head. You can print or the doctor himself prints the certificate. After which he will sign on the certificate and stamp will be required.
