Yes,It is mandatory to have name and date on the photograph for the 2020 NEET application and also the photograph should be covering 80% of face with ears clearly visible and format is in JPG or JPEG only . You can find more details in the below link.

Yes photograph with name and date for neet 2020 application form is necessary and here is few more details whuch you have to keep in mind while uploading image.
Passport size photograph should be in JPEG/JPG/GIF/PNG format and 50-100 kb of
3.5X4.5 cm dimensions
Signature-
JPEG/JPG/GIF/PNG format ,10-50 kb,6X3 cm
Left-hand thumb impression
JPEG/JPG/GIF/PNG ,10-50 kb.
