The documents required to change name in 10th marksheet marksheet are:-

Affidavit describing that you are seeking for a name change and also you have to explain the reasons for changing your name.
Publish the change of name in at least two local newspaper and keep proofs with you.
A copy of your present identity (PAN card, voter ID card, passport, etc).
