Hi Vinod!
Apologise for the prior answer somehow it got entered.
So for that you need to get a PWD certificate from the district hospital where you would be examined.
The examination will be examined by a group of doctors and after the analysis,the hospital would provide the candidate the certificate with percent of disability.
And according to that you could use it for allocation of seats or filling the forms
