see some times during document verification, college or exam authority check the 10th mark sheet, and 10th mark sheets name and dob should be follows in every form filling whether it is for entrance test or job recruitment.  exams like ssc, upsc. and most of the form filling for pvt or government it is mandatory to give first name and surname according to 10th mark sheet wise. I think from on wards you need to mention your surname also. so do the form filling including surname. however you may contact with your institution to provide a duplicate certificate where first name and surname both will be mentioned.
