You can fill out the MNS form even if you wear spectacles. Here are some of the key factors that you need to know:

Myopia - Should not go beyond -5.50
Hypermetropia - Should not go beyond +3.5.
With Glasses, the better eye should have a vision of 6/6, and the weaker should have a vision of 6/18
