Dear Harsh, Physical Disability has no vertical Reservation. It is a horizontal reservation within the Category your brother belongs to. For example, if he belongs to General Category,he woll be considered under General Category-PH quota. Now the cut off for General Category is 134-701. And PH gets a cut off 45% which means the cut offs will be lesser than his range. Now not every qualified candidate can get a Medical seat. Okay..Is there another list present for the disabled candidates other than actual merit list of candidates?

No. Its the same merit list. Ranks are given keeping in mind the score, reservation.
