My daughter is 40% Handicap in visson parmanant. so what is process for neet form fill up.

You will have to select the Category you belong to and then also select PWD as Yes. For example, if you belong to General Category, you have to select General Category. And then where it asks PWD Status, you should say 'Yes'. PWD reservation is not a vertical reservation but a horizontal reservation within your Category.
